Ingame name: SHAz_181 (With tag: =[BF]=SHAz_181, but I didn't have the tag at the time)
Date: No idea, but it was well before the release of 0.8
Offence: I'm not entirely sure. It could have been tking but I sincerely doubt that (I never purposely TK). It was more likely to have been swearing. I think I got auto-kicked for that, but it might have buggered up or something and banned me.

All [T&T] servers have 3 banlists - modmanager, Punkbuster, and PB Master Ban List.

I find no reference to SHAz_181 on any of them.

I've searched by GUID and HASH.

As I said in the realitymod.com thread, you may have tried to connect whilst the server was full. It's unlikely to be busy right now, so try to connect.
